IV期下咽頭がんは IVＡ期、IVＢ期、およびIVＣ期に分けられる。IVB期では、腫瘍は（１） 隣接する軟部組織、結合組織、血管、甲状腺あるいは食道に拡がっており、大きさに関係なくリンパ節に拡がっていることがあるか；あるいは（２） 大きさは様々で、リンパ節に拡がっており、そのリンパ節の大きさは6cmを超える。
