がんが（１）腎臓周囲の脂肪組織層を超えて拡がっており、周辺のリンパ節１つに見つかるもの、または（２）周辺のリンパ節２つ以上に拡がっているもの、または（３）腸管、膵臓、肺などの他の臓器に拡がっており、周辺の複数のリンパ節に見つかるもの。「stage IV kidney cancer（IV期腎臓がん）」とも呼ばれる。
