がんは食道内壁細胞の最内層にみられる。「carcinoma in situ（上皮内がん）」とも呼ばれる。



食道がんの病期分類；0期、I期、II期、III期、IV期の食道がんで、腫瘍が食道壁の各層を貫通して成長していく様子を示す。拡大図は、食道壁の筋層、粘膜下層、粘膜層と、リンパ節および血管を示す。
食道がんが0期からIV期まで進行するにつれ、がん細胞は食道壁の各層を貫通して増殖し、リンパ節や他の臓器に拡がる。
